Medicare entitlement and payment order are different questions. Understand both before changing coverage.

Two different questions
When can Medicare begin? The treatment dates help estimate entitlement. Who pays first? That depends on your other coverage, Medicare history, and the 30-month period. One date does not answer both questions.
“Pays first” means that plan processes a covered claim first. A second plan considers remaining covered costs; you may still owe a portion.
The 30-month coordination period
During the 30 months
The period generally begins in the first month a person qualifies for ESRD Medicare, even if they enroll later. For employer or union group coverage and COBRA, that plan generally pays first during the period when ESRD Medicare is the first Medicare basis. Medicare can pay second if it is active.
After the 30 months
After the period, Medicare generally pays first if it is active and ESRD entitlement continues. The group plan may pay second if it remains in place. If ESRD-only entitlement ends sooner, the 30-month date alone does not create Medicare coverage.

The first ESRD entitlement month is month one. Month 30 ends on its last day. Changing group plans does not by itself restart the clock. A genuinely new ESRD entitlement after termination may have its own period.

Employer, spouse, retiree coverage, and COBRA
Employer or union coverage can be through your work or a family member’s work. COBRA and retiree coverage have their own enrollment consequences. Ask each plan’s administrator how it will pay before and after the 30-month date, especially if Part A or B is not active.
Enrolling in Medicare may affect COBRA rights depending on which coverage began first. Do not assume a plan remains in place unchanged.
Medicare through age or disability
If Medicare was already active because of age or disability before ESRD eligibility began, do not assume employer coverage becomes the first payer for a new 30-month period. The payment order already in place may continue, depending on the group plan and Medicare history. If ESRD-only entitlement later ends, Medicare through age or disability can continue under different group-plan rules.

Other coverage
Medicaid
Medicaid generally pays after Medicare and other health insurance for covered services. Ask your state office what it covers. Find your state Medicaid agency.
Marketplace
Marketplace coverage does not end automatically when Medicare starts. Update the application after confirming Medicare dates; premium tax credits may need to be repaid.
VA benefits
Medicare and VA generally do not pay for the same service. Which program applies depends on where and how you receive care.
TRICARE and other plans
These plans have separate coordination rules. Confirm the order with the plan and Medicare before changing coverage.
